Counseling Rates

Initial Assessment & Evaluation: $125.00

The initial assessment will be a diagnostic evaluation. In this evaluation, KJ will gather personal information for the purpose of determining issues that need to be addressed and recommendations for how to address them. Goals for treatment will be discussed and agreed upon by both the counselor and the patient(s). This session will be 50 minutes in length.

Individual Session: $110.00

The frequency and duration of counseling will be determined based upon the patient’s individual needs and will be discussed at the initial assessment. Individual sessions will be 50 minutes in length.

Family/Group Session: $110.00

The frequency and duration of counseling will be determined based upon the patient’s specific needs and will be discussed at the initial assessment. Family and couples sessions will be 50 minutes in length.

Insurance

Currently, I am not an in-network insurance provider, and I do not file for insurance payment or reimbursement. However, for most insurance plans I am an out of network provider. I can provide you a receipt if you wish to submit it to your insurance company for reimbursement. In addition, I do accept health savings account cards.

Payments

Payment is expected in full at time of service. I accept cash, checks, credit, debit, and HSA cards (Visa, MasterCard, American Express, and Discover.)
